(Failure to complete this form each Friday by midnight indicates your resignation and cancellation to receive any future commission payments.)

TERMS & DELIVERY 
TERMS:  50% deposit required to commence order, balance due on completion PRIOR TO DELIVERY - no billing option available.

DELIVERY:  Average turnaround time  for completion is 30 days from date of of orders under 5,000 cards.

AGENT COMMISSION STRUCTURE 
Sale 1-3 : $500 per sale PLUS .15 per card
Sale 4-6 : $750 per sale PLUS.20 per card
(Then only 1 sale per month to maintain this rate)
Sale 7+ : $1,000 per sale PLUS .25 per card
(Then only 1 sale per month to maintain this rate)

MANAGER COMMISSION STRUCTURE 
$500 per sale PLUS .10 per card

SETUP FEE 
Setup Fee ($6K) Includes the following features and is only due on the first order - additional orders (with the same printing and content) do not have a setup fee:
